LEARNING LOGS

Learning Logs are a form of meeting minutes. A Learning Log is made for every consultation/meeting with a Facilitator. It is also made to track my progress, reflect and record key points discussed in consultations, for each of the four stages: Plan, Perform, Monitor and Reflect.
